• Do you have any of the following symptoms: sore throat, flu like symptoms, headache, vomiting, a cough, shortness of breath, sneezing, runny nose, loss of smell or altered sense of taste? 
• Have you, to your knowledge, been in contact with a confirmed or probable case of Covid-19 in the last 7 days?
